Mutual of Omaha Transmittal Form Changes - Effective 9/23/2013

Hi ,

Effective 9/23/2013 Mutual of Omaha will have NEW contracting
forms, and any older versions will no longer be
accepted by the carrier.


Due to changes in the transmittal forms for the following carriers:

· Mutual Of Omaha Ins Co
· United Of Omaha
· Omaha Insurance Company
· Omaha Life Insurance Company
· Companion Life Insurance Company of NY


Along with any other affiliated companies, SureLC will be making
some adjustments to the Compensation Tables for each of
these carriers.


If you have previously set up commission schedules within SureLC
under any of these carriers, you may want to ensure that you have
this information "saved" prior to SuranceBay making these
adjustments.


To Save a copy of what you currently have on file......

· Retrieve a copy from an old contract request

· Or you can create a NEW request to retrieve this data.

* Go to any producer with a DBA setup as an
"Individual" or "Business Entity".
* Request an Appointment
* Ensure ALL product Lines are selected in step 2.
* Select a schedule in step 4
* Select the Administrator checkbox in step 5 so the
contract is only sent to yourself.
* Process the request.

IMPORTANT NOTE: You'll want to repeat these steps for each
schedule selection you have created in your system.


What will be changing to the Compensation Table?......

The carrier has simplified the entire contracting package including
the Transmittal pages. Currently there are separate transmittals
for each product category. With the new format everything will be
condensed to 2 transmittals. One for Life and Annuity products and
one for Health products.


Where can I get a copy of the Transmittal?......

Mutual of Omaha has asked that all transmittals be provided by
request only. There are different transmittals assigned
based on your contract established with the carrier.


** What this means is that your agency will need to send
SureLC a copy of the required transmittal as verification of the
Transmittal version needed. **


Contact MUTUAL OF OMAHA if you are not certain which transmittal
applies to your Agency's contract.


Once you've retrieved copies from the carrier, simply email them
to: support@surancebay.zendesk.com along with your Agency's Name.


This is a one-time request. Once we've set your agency up with
the correct versions needed they'll be available for all future
contracts processed.

SuranceBay plans to make these adjustments effective Monday
9/23/2013 by 8:00am EST.
