Monumental Life Ins Co - Old forms reverted back temporarily

As many of you are aware, SuranceBay updated forms for Monumental
Life Ins Co in order to switch over to the newly branded name
Transamerica Premier Life Ins Co, with the effective date of
8/1/2014. This was done so at the request of the carrier.
However, it's come to our attention that Transamerica (parent
company of Monumental Life) needs to delay this changeover until
9/30/2014. For that reason, we've put back the old carrier
selection along with the old forms. For those agencies working
with Monumental Life Ins Co, you now have access to the necessary
contracting forms as you had prior to the 8/1/2014 update.
Transamerica Premier contracting forms are ready to go and will be
put back into production once confirmation (on or around 9/30/2014)
from the carrier is provided to us.
If your agency works with Monumental Life Ins Co, you will need to
add this carrier back into your SureLC assortment of carriers.
